We accept most PPO insurance plans. Patients with PPO insurance can see us without needing a referral or authorization.

- PPO Insurance Approval Process
Obtaining an insurance approval for weight loss surgery can be a difficult process. The first thing we do is call your insurance company for two things: 1) to verify that weight loss surgery is a covered benefit, and 2) to see what the exact requirements are to obtain authorization for surgery. We will then help you to meet the necessary requirements.

- PPO Out of Pocket Costs
There often are out of pocket costs associated with having surgery. Out of pocket costs can include things like program fees, deductibles, and co-insurance.

We generally do charge a program fee and (if applicable) your insurance deductible. The program fee covers the non surgical elements of our program, including our educational program and our aftercare program. These program elements are necessary to help patients reach and maintain their weight loss goals. Insurance companies generally do not pay for these costs.

HMO Insurance
HMO patients need to be referred by their primary care doctor to see us. Your primary care doctor may or may not be able to refer you to us, depending on whether or not we have a contract with your HMO.
