The first step is to call our office 714-582-2530 or send us an inquiry form so we can contact you.
We will schedule you for a weight loss surgery seminar so you can learn more about surgery, and inform you of the insurance approval process for your specific policy.
Weight Loss Surgery Informational Seminar
Our informational seminar is a free, no-obligation presentation on weight loss surgery given by Dr. Oliak.
The purpose of the seminar is to give you an overview of weight loss surgery, including benefits, risks, and alternatives, so you can better decide whether weight loss surgery is right for you.
Seminars are regularly held in Brea, California, and Orange, California.

Health Questionaire and Insurance Card
The next step is to complete the health questionnaire and send us copies of your insurance card. You can download the health questionnaire here. We use the questionnaire to help determine if you are a candidate for surgery and to help us obtain an insurance authorization for surgery.
Obtain Insurance Authorization for Surgery
To obtain an insurance authorization you will need to do all the things required by your specific health insurance policy. Examples of common requirements include psychological evaluations, nutritional evaluations, medically supervised diets, documentation of medical problems, etc.
We will help you to complete the items your insurance policy requires so that we can then request an authorization for surgery. Once we have sent all required documentation to your insurance company, they will generally approve surgery within one month.

Preoperative Evaluation
After insurance authorization is obtained the preoperative evaluation appointment will be scheduled. You will see the doctor for a history and physical so the appropriate preoperative tests can be ordered. Required preoperative tests generally include blood and urine tests.
At times patients need to be evaluated by specialty physicians (cardiologists, pulmonologists, etc) and undergo additional testing such as stress tests, pulmonary function tests, and x-rays.
Patients are generally given a surgery date during their preoperative evaluation day.
Office Day
After the preoperative evaluation is completed you will be scheduled for an office day. The main purpose of this day is to make sure you have a good understanding of surgery (benefits, risks, necessity of behavior modification), and to review the test results from your preoperative evaluation.
You will attend education classes, take a written test, and meet with Dr. Oliak.
